This presentation summarises the evidence-based clinical recommendations for orthodontic treatment in patients with Stage IV periodontitis, as outlined in the EFP clinical practice guidelines.
Emphasis is placed on biological safety, timing of orthodontic tooth movement, and periodontal monitoring before, during, and after orthodontic treatment.
The stepwise interdisciplinary workflow, from initial periodontal therapy and re-evaluation to orthodontic and restorative rehabilitation, is illustrated through a clinical case. The case demonstrates how orthodontic treatment, when performed under strict periodontal control, can safely correct pathological tooth migration, improve function and aesthetics, and contribute to long-term periodontal stability.
The presentation highlights the importance of lifelong supportive periodontal care and close collaboration between orthodontists and periodontists in the management of advanced periodontitis patients.

Learning Objectives

After this presentation, participants will be able to:

  • Understand the biological principles governing orthodontic treatment in patients with Stage IV periodontitis
  • Identify periodontal endpoints required to safely initiate orthodontic tooth movement
  • Apply a stepwise interdisciplinary workflow for orthodontic and restorative treatment in advanced periodontitis patients
  • Recognise the importance of periodontal monitoring, retention, and long-term supportive care to ensure treatment stability
